Registration Form for SBU Math Contest
Before registering any student names, be sure to fill out the advanced reservation form for space at the competition:
For October 26, 2019-please enter the students' names and grades directly into the database, if at all possible. The link to do this will be provided by late September, 2019, emailed to those schools making advanced reservations. Students and schools must be registered BEFORE the contest. Students arriving at the contest without the school already registered will not be allowed to participate.
You MUST register your school for 2019-20 first at http://mathleague.org/membership.php before you can register students for the competition.
For 6th graders, be sure you register them for the intended level. They are eligible to participate in either level, but can only do one level at this competition.
Elementary (grades 4-6) the student registration link is http://mathleague.org/register0.php?event=5427.
Middle School (grades 6-8) the student registration link is http://mathleague.org/register0.php?event=5426.
This contest is independent, but Missouri students participating can still qualify for the "next level" of MCTM sponsored math contests. For Elementary, the next level is a regional contest in March (or early April) and you can find more information at https://www.moctm.org/ElementaryContest.aspx by October, 2019. For Middle School, the next level is the state contest (date TBA) and you can find more information at https://www.moctm.org/MiddleSchoolContest.aspx
If necessary you can still email or mail the completed registration form below, but I would prefer you do the name entry directly into the database.

Copy this form if you are registering only part of the students from your district (or over 20 students). Please share this information with other teachers who might be interested. For sixth graders be SPECIFIC as to which competition they are registering for. This won't be a problem if you use the online entry links above.

Extras For Cafeteria = ______ x $7.50 each = ______ total additional cafeteria payment due (can be made the date of the contest when actual numbers are known or can be paid at the cafeteria entrance-but cost will be slightly higher at entrance). Any parent or sponsor entering the cafeteria with their student is assumed to be eating and will need a meal ticket. Purchasing lunch at the door will be $7.95 when tax is added.
Cost is $12 per student, which covers both the contest and lunch in the SBU cafeteria. We hope this cost does not prohibit any interested student from participating. Checks should be made payable to Southwest Baptist University - Math Department. Purchase orders are acceptable or you may pay at the contest itself.
