Safety and Photo Release Waivers

Attached to the page below are files with the Mad Scientist Day Safety Waiver and Photo/Video Release Form for 2020. We ask that you please bring these signed to the event as it will make your sign in time much smoother and allow you to get to the fun faster! We will have extra copies at the sign in desk in case anyone forgets to bring them, however we will only have a limited number.

If an adult other than the child's legal guardian brings them to this event, these forms MUST be signed in advance with consent from the legal guardian.

In order to print the forms below, scroll to the top right corner of the document where you should see the pop-out icon. Click on the pop-out icon which will take you to the page to view the document. From here you can select either the download or print option located in the top right corner of the page.
