Appointments:
Due to the nature of a retina practice, please be prepared to spend one to three hours with us for your visit. Dilation of the pupils requires time and we may preform specialized diagnostic tests during your visit. After your exam, there will be a detailed discussion of your condition and treatment options. Follow-up appointments will be scheduled as necessary. If at any time you have a question regarding your condition or any testing we have done, please call and ask. It is our intention to educate our patients. Please ask rather than worry.
Please bring the following with you:
- Eyeglasses currently being used
- A list of your medications, which you are currently taking, including their dosages and how often you take them
- Names, addresses and phone numbers of all your current physicians
- Previous medical records relating to your visit or any information relating to your eye condition
-For diabetic patients: A list of your recent blood sugars and most recent Hemoglobin A1C
- All of your current insurance cards. If your insurance is and HMO policy, it will be your responsibility to obtain a referral prior to your appointment.
- Because your eyes will be dilated and it may be hazardous for you to drive, you should bring an alternate driver
- An e-mail address. If you do not have an e-mail account yourself, please bring the e-mail address of a trusted individual to whom we may send information such as summaries of your visits and appointment reminders.
