Each team member must be duly registered as a PCMCA member by January 10th, 2022 be 60 (a and b) or 70 (c) years of age prior to December 31, 2021, and play in a weekly scheduled daytime ‘draw from a hat’ PCMCA league at the same club.

Submit this form, by January 4, 2022, to your club’s playdown coordinator who shall send a copy/e-copy to the Zone 2 Senior Director by January 10, 2022.

I declare that the curlers named above all curl for the club in a PCMCA league, have paid their PCMCA dues, have correctly shown their birth year, and have paid in full their annual curling club dues by January 8^th, 2022.

Club PC: By January 8, 2022 each club must: i) determine a club team winner, ii) declare a 5^th player, iii) email this information to the Zone Senior Director. The Club team winner must pay a Zone entry fee of $160 (to be submitted to the Zone Director on day 1 of the Zone playdown. The Zone Director will forward the Zone fees and the Zone winner information to the PCMCA secretary/treasurer as soon as possible after the completed Zone playdown.

Note: The host club may charge ice fees to offset hosting the Zone playdown. Revised Sept 20, 2022
