Below are forms required for unit outings and camping events. A Parent Permission is required to be on file with the Pack. Use the Activity Consent form for camping events with District, Council or other units (i.e., Boy Scout Troop). The Health & Medical form 680-001_AB is required to be on file for participation in all unit outings. A separate medical form is required for all adults and youth attending any over night outing of less than 72 hours (copies will not be returned). DO NOT SEND MEDICAL FORMS VIA EMAIL OR OTHER ELECTRONIC MEDIA! IT IS A VIOLATION OF BSA POLICY.
