NOTES ON FILLING OUT THIS FORM:
Text boxes can hold more text than you can see in the box – just keep typing if you need to.
If you need to change your application later, please fill out an entirely new form including a note in the last text box to REPLACE your previous entry, which we will do manually.
Please submit one form per person (kids as well as adults)
NOTE: seems like every now and then this form just doesn't accept an application... we're not sure why. For your longer answers you might type them into a text document and then copy/paste them into the form just in case you have to do it twice... if you have problems please send an https://sites.google.com/site/notc2012/ email... thanks (and sorry if it doesn't work - please don't give up - try a different computer/browser/time if you have to...)
