TRYOUT INFORMATION

All Paper work must be completed and turned in to coach Mark prior to start of Tryouts.

The following is a list of what must be completed.

PINK Slips:

Please follow the directions listed below to get your pink slip.

On the school's Athletics web site, under the A.D's Picture in the left column are the forms that need to be completed for your Athletic clearance (pink slips)

1. (Physical form) is the paperwork needed for your YEARLY sports physical. Print out a copy and take it with you to your doctors visit when going for your sports physical.
2. (Parent consent risk form) is the paperwork needed to complete your YEARLY athletic clearance. Please have your parents complete and sign this form.
3. Then turn the COMPLETED Physical and Parent consent risk forms, ALONG with the $10 Processing Fee (checks Payable to Moanalua High School) in to the athletic trainers office to be processed.
4. Pick up completed Athletic Clearance (pink slip) from the athletic trainers office ONE DAY AFTER turning in COMPLETED physical and parent consent risk forms and processing fee.
5. Turn in Pink slip to coach Mark so you can practice and compete!

White Slips:

Will be distributed ONLY at the mandatory parent meeting to be signed and returned as proof of attendance.
