Supplemental Applications

If you are already a member and would like to prove descent from another Mayflower passenger, please submit a Supplemental Preliminary Application and Lineage Review form. 

Mail the two  forms along with your application fee to the address on the forms.

Please follow the instructions on the forms.  The Supplemental application fee for present members is printed on the form.

The fee is due at the time of application and is not refundable.

Transfer In

If you would like to transfer in from another Society, please complete the Transfer In From Another Society Form.

Reinstatement

If you have been dropped from the membership rolls and wish to be reinstated, please complete the Reinstatement Form.

Dual Membership
