Science Congress Approval Form
Required for Every Student!
Click on "2024 LISC Approval Form" above to download this year's approval form. Make a copy for EACH STUDENT who should complete the form with signatures, then return the signed form to the liaison.
Liaison will:
Compile all science congress approval forms into one PDF document, alphabetical by student last name.
Name the PDF file “school name_LISC approval forms 2024”
Email the one PDF document to the Division Director by February 28th, 2024.
*NOTE - liaison's email of the approval forms is the indication to the director that the student project data entry forms are complete. Please send the PDF after all final data has been entered online, but before the February 28th, 2024 deadline.
*It is recommended that you complete this process a few days BEFORE the deadline so that if mistakes are made, there is time to correct them.
