Important Documents, Information and Payment/Refund Deadlines

1. To register for the trip:

Please download/fill out the registration form below. Be sure to sign it, then mail it and a $300 deposit per person to our travel agent at:

2. Regarding your Passport:

If you don't presently have a passport, fill out and send in your registration now anyway and simply forward on the passport information later when you do have it. NOTE: If you DO have a passport, check the expiration date. It MUST expire later than 6 months after the last day we are in Israel. If it doesn't, apply for a new one now. It takes about 6 weeks to get a new passport, so acting now will prevent feeling rushed later.

There is an expedited process for a fee (to the passport office) to receive a passport in less time. Your local post office will have passport applications, and any Walgreens or like entity, will take passport photos for you on the spot for a nominal price.

3. Payment and Refund Deadlines set by the travel agent due to costs they incur:

• Final payment on the full trip cost is due by December 27, 2011
• Full refund: If you cancel your participation by 120 days before departure, which means by and including November 28, 2011
• $100 charge: If you cancel your participation by 119-90 days before departure, which means between Nov. 29 and Dec 28.
• $200 charge: If you cancel your participation 89-61 days before departure, which means Dec. 29-Jan 27.
• No refund if canceling 60 days or less before departure which means January 28 or later. (But someone can go in your place if you can arrange that.)
