With other wonderful resources and advocates in the field of dyslexia, I feel it best to consolidate resources rather than "reinvent the wheel." It's so important that we, as a community support each other and know what resources are available from those who have worked so hard and diligently!! On that note, I want to direct you to Decoding Dyslexia--Arizona. They are "A parent-led, grassroots movement promoting the identification, education, and support of students with dyslexia in Arizona."
