A depository for forms and templates that people have contributed.

PDFs can be printed and used as-is.  The XLSX and ODS files have templates that can be edited.  For the ODS, the image FORM BACKGROUND.PNG must be inserted into the first cell.  The XLSX file has this background image in the header.  

The PPT file has all bubbles that are in the forms.

form-test-macro.ods takes user date and creates a pre-compiled form with student data.

Did you know that you no longer have to use the big circles as markers.  Change your marker type under Settings to "Angular."  Here are some forms to get you started.
