This form was created for a global health course to allow students to select a research topic, a book to read, and to select a time they preferred to do their final presentation. I used a Multiple Choice Grid for the time selection which allowed the students to make a preference for each of the time slots.

This form was created to register participants for a professional development course. The header image was made using Photoshop incorporating two images, coloring the background, and inserting a text box.
