Instructions for Fillable Team Roster:

Click the Download link above, the form will open in Google Docs
Click the Ellipses (three dots) to the left of the Share button
Click Download again, the form will open in Adobe Acrobat on your local computer
Click the A in the dotted box icon in the left menu, this will allow you to type your roster into the form fields
Save and print! Open and reuse for the next match.

Instructions for filling out Captain’s Score Sheet:

“Club 1” will fill in A1, B1, C1, D1. Club 1 is the club listed first in the pairings, i.e. Blackhawk (club 1) vs. Discovery Bay (club 2). “Club 2” will fill in A2, B2, C2, D2.
Score sheets must agree in all ways and then be signed by captains. “Attest 1” is captain from “Club 1” and “Attest 2” is captain from “Club 2”.
Each team captain will send a photo of the signed score sheet to: Donna Grover 408-981-6073

Word Version: Click on the Bag Tag below and double click "Your Club" to edit the text.
