• BTR FORM 1 - Guide on how to fill out the new version of BTR Form1

• BTR FORM 2

• BTR FORM 3B

• ANNEX D

4. All Encoded

5. All Signatures must be OVER Printed Name

6. BTR Form 2 and BTR Form 3 MUST be Notarized

7. Submit to Cashier Section
