Since they are not, perhaps it is better to analyze mo-/mbo- as a prefix used to introduce a causee/agent into a patientless semantic structure.
Page 216: "becomes a cause marked as a direct object". This should read "causee".
Page 221: The graph for "Derived ditransitive verb" is incorrect, since the top line has "(Indirect object marking)" and "Direct object marking" reversed.
This is because the A argument (the participant made to do something to a patient or "causee") is most often omitted, but when it appears, it is marked as an indirect object. The P participant (the patient that receives the action or last participant in this causation chain, the "affectee") is often expressed and is marked like a direct object, that is, unmarked if it is inanimate, or marked with =pe if it is animate. The correct graph should look like this:
