Here's is Theresa's explanation for each of the IRB docs:

1. Forms A and A-1 - just listing the title of the study and naming members fo the research team - pretty self-explanatory

2. Form B - Research description - need to clearly describe the study, using the headers provided by the IRB. Carefully explain the protections you are providing for the participants - how you will protect their responses, the data (should be saved in password-protected file), their identitiy, etc. Language in the LINKS slides will be very helpful for network studies.

3. Form C - Informed consent - follow the format set by the IRB and carefully explain the study in language that will make sense to respondents.

4. Form F - Waiver of documentation of informed consent - if you're doing an online study, IRB usually approves this form. All you have to do is make sure that they have to indicate consent before they can answer the survey questions.

5. Form L - if you're doing any recruitment of subjects, need to turn that in for approval

6. Form M- survey - I usually just download my SurveyGizmo survey into word. Doesn't have to be "pretty" they just care about the wording.

7. Form Z- signature sheet. Only a PhD can be the principal investigator unless it is a doctoral candidate doing their dissertation research. Dan B. also signs off on all our studies.

8. Cover letter - feel free to use it as a template. Should be included in packet that goes to the IRB.
