What is Google Forms? Google Forms is a free tool from Google that allows you to do the following:
Create forms, surveys, quizzes, and such
Share the forms with others
Allow others to complete the forms online
Collect all the responses in a spreadsheet
Provide helpful summaries of the collected data with charts and graphs
Google Forms can be accessed through the Google Drive screen or the Google Forms web app. Anyone can use Google Forms with their personal Google account, or through a Google Apps for Education account.
Are Google Forms only for surveys? Not necessarily! Google Forms can be used to plan a camping trip, manage event registrations, whip up a quick poll, collect email addresses for a newsletter, create a pop quiz, and much more.
How can Teachers use Google Forms?

Parent Survey- Gather information from parents about their contact information, information about their child, volunteer opportunities, etc.
Science Fair Judging Form
Booklist Template - Used by teachers to keep track of the names and quantity of books read by students.
Discipline Entry Form - This form allow teacher entry of disciplinary actions. Note: We use avatar names when entering information.

How can teachers create new Google Forms? Once you are in Google Drive you can create a new form as follows:
Click the New button in the top left corner.
Then click More from the drop-down menu.
Finally click Google Forms from the pop-up menu.
Your blank form will now be created.
To name your form, type the title in the box where it currently reads Untitled form. You can always change this later.
Your form is now ready for you to edit.
Choose from tons of new and ever-expanding themes or use your own photos, company or school logos, and color palettes to create your own unique form. Invite an unlimited number of respondents to your form by sharing the URL, emailing an embedded form so they can respond without leaving their Gmail inbox, or adding it to a website.

The survey that I created below is to find out if our students have access to technology and internet at home. I always wanted to create an online school so that students can have access to the curriculum anytime and anywhere. We could include digital books, tutorials, videos, and websites for fun and practice. All the work could be posted online and parents could help their children with the homework without much frustration. As a future administrator, before I would continue towards my plan, I would let the parents take this survey to find out how many students do not have access to technology at home. I would definitely provide an alternative to those families.
It was very easy for me to create this form through Google Forms. This is just a sample form that I created to see what kind of data I would collect through this process. I attached a copy of the data below to show how it looks like. After I sent my form, I needed to specify a spreadsheet for viewing people’s responses. I was prompted with two choices:
1. New spreadsheet: This option is to view responses in a new Google Sheet. This is the default option. I chose this option.
2. New sheet in an existing spreadsheet: This option is to view responses in an existing spreadsheet already in my Google Drive.
I download the data collected through the survey feedback by selecting File > Download as > Comma Separated Values and uploaded that file at the bottom of this wiki page. These responses could give me specific information about those students who do not have any access to internet outside of the school. I also looked through the summary of responses by selecting Form > Show summary of responses. I could see specific information about each response with graphs, which was very neat.
After collecting data, there should always be an action plan. My action plan; as a school, we would decide if having a virtual school would benefit our school community or not based on the data collected through Google Forms. How are we going to provide the same standard of education to those students who are economically disadvantaged?
As a future school administrator, I would use Google Forms to get feedback on school initiatives, curriculum night or any school event, school management, technology issues, and professional development activities. I can also use these forms to track students’ disciplinary referrals and classroom walkthroughs. The schools need to provide various opportunities for students, teachers, parents and other community members to give a quick feedback on the students’ learning process and school management. That can be done effortlessly through Google Forms.
