Services

Advanced Living Care, LLC provides non-medical home-care services to adult clients age 65 or older and children ages 5 or older with physical and developmental disability who require assistance in their own home to assist in the maintenance and retention of their independence and well being due to medical conditions. These services are responsive to the distinct needs of the clients and are delivered by qualified personal care attendants.

Service Geography

We provide services in the following counties:

Barron, Chippewa, Dunn, Eau Claire, Pepin, Pierce, Rusk, and Saint Croix.

Clients who are not within the Agency’s service area will need to be evaluated further before accepted for service.

Clients reside out side of these counties are encouraged to call us. We are happy to give referrals and provide other assistance to accommodate your personal care needs.

Scope of Care

There are four main categories of personal care assistance our provider offers.

ELIGIBILITY CRITERIA

In order to receive personal care service from Advanced Living Care, clients must be a Wisconsin resident and meet one or more of the following criteria:

- Age 65 or older with physical and/or developmental disability.
- Age 5 or older with physical and/or developmental disability.
- Be eligible to receive Medical Assistance.
- Must have medical needs and personal care service is ordered by a physician.
- Be able to make decision about their own care or have someone who can make the decision for them.
- Be eligible for Wisconsin alternative care and waiver program.

SERVICE DELIVERY PROCESS

Any person may request personal care services from Advanced Living Care. Request for services and/or information may come by way of phone, email, regular mail, fax, website, in person or from a number of other sources.

1. Request for service
2. Initial home visit to complete admission forms
3. Arrangement for an in-home nurse assessment
4. Physician order authorized for personal care service
5. Prior authorization
6. Orientation and sign Service Agreement
7. PCA is arranged and scheduled to provide care service
