Junior High Boys Basketball

Physical Forms are due before you are allowed to start practicing.

If you had a physical done for football or cross country, you do not need another
No physical = no practice

All players will also need to complete & turn in the "Heads Up" concussion form before our 1st competition date.

If you filled a concussion form out for football or cross country, you do not need to fill another form out.
No concussion form = no game play.

* PARENTS - I will be looking for pictures from our games to post on this website. If you take pictures during the games, and you email them to me, it would be appreciated. Thanks, Coach W cwildeman@siouxcentral.org

Parent Information:

Player/Parent Agreement should be filled out by the parent and the student together. One form per player.

- Click on the player/parent agreement link, then a google form will open

- Click on the link to the document, doc should open up in a new tab

- Read through the contract together

- Go back to google form tab and click next, answer the questions in the google form and then submit

Rooms through the SC app

Communication will be done through the rooms app (SC app)

I will create a Room with all the bball players
Parents are automatically added to the room - as long as the school has an email for you in our student information system.
Download the SC app on your phone & then sign in with the email the school has on file.
- It must be the email the school has on file or it will not connect you to your student.
