To change your details at the GP:
Preferred name and waiting room calling system
This does not require any documentation. You can simply ask any GP to change your preferred name on the system and they will do this for you.
Legal name
Simply give your deed poll to a receptionist and tell them you wish to change your legal name with the NHS and they can do this easily then and there.
Title (and gender)
You can change your title to ‘Mx’ without any legal documentation, but to change it to a different gendered title to your AGAB title, you will need to change your gender with the NHS. To do this you will need to speak to a GP. This will also require the creation of a new NHS profile for you, including a new NHS number. To ensure all of your previous record information is transferred over, you need to bring this up with your GP and follow their advice if other steps/communications are required.
Different areas of the NHS do not communicate well (if at all) so it is highly likely that it will take far longer for your name to update on the system in hospitals (legal name only) and may not be correct at first on certain referrals.
