Mentor Agreement Form:
All mentors complete and return to SMC prior to being matched with a mentee 

Parent Consent Form: 

Prior to matching with a mentor, the parent must sign the consent form for their child to have a mentor.  

Updated consent forms are required for the new school year for ALL mentors.
