We need 5 things to complete our mission:

Add a folder to your Google Drive and give it a descriptive name. This is where the merged documents will stored.
Create a spreadsheet with reliable data. See below for a sample.
Create a form that asks your questions. See below for a sample.
A template that will be merged with the information submitted to the form. A template for autoCrat can be a document or a slide. See below for a sample.
And you need the autoCrat add-on. See below for a link!

Get autoCrat

Under the Add-ons menu, choose the last item, "Get add-ons."

Give it permission to run by choosing "Launch."
