The Youth Brain Injury Program offers custom documents to meet the diverse needs of Alaska educational teams supporting youth returning to school following a brain injury. These documents were designed with inspiration from outside programs and technical assistance from the Oregon Center for Brain Injury Research and Training (CBIRT) to be inline with current best practices.

We encourage all teams to use this document to improve consistency and effectiveness of temporary student support plans for all students returning to school following a brain injury.

Teachers are encouraged to use this document to document and keep track of the accommodations being utilized in their classroom and provide feedback on the students progress. The feedback document will assist the team to adjust accommodations on a weekly basis or provide the team with suggestions if needed to move forward to a 504 or IEP.
This document is unique and designed for the student to complete themselves, or initiate conversation with the student about their symptoms thus moving toward goals to resolve such symptoms and progress monitoring.
