My name is ______________, a current Grade _______  DP student. I’m planning to do a CAS experience called “[Experience Name],” and I was wondering if you would be willing to be my CAS Supervisor.

The experience will involve [briefly describe what you will be doing]. It will run from [start date] to [end date]. I will be writing a minimum of: 

• 3 reflections on this experience and/or

• 5 reflections for this project are organized by the following stages: Investigation, Preparation, Action, Reflection and Demonstration. 

• I will also upload a minimum of 2 - 3 pieces of evidence (photos, videos, notes) to detail my learnings, growth and participation in the said experience. 

I would really appreciate your support and guidance throughout the experience.
