Returning Student Packets

In an effort to streamline returning students packets travelling between school and home, please review the following forms, complete and/or read them in order, and take note of important information in preparation for the 2020-2021 school year. More information will be coming soon related to the COVID-19 Addendum to the Student/Parent Handbook, our initial plan for reopening in September, and any other important updates. The currently published 2020-2021 school calendar may change; we will communicate any changes by announcement and a formal publishing of a new schedule if need be.

Please read through the following letter, outlining all the components to the Returning Students Packet. Please understand that many of these forms and consents have been condensed into easy-to-fill Google Forms that are available in the links below. What is important in this letter are the full-day hours and tentatively scheduled half- and full- days related to professional development - these are subject to change as the schedule may change this year.

Part Two - SEEM Prep Student Emergency Information

Please go to this Google Form link to complete your child's personal and emergency information, medications, medical needs (allergies, seizures, asthma, et cetera) and releases (over-the-counter medications, communication between school and physicians, et cetera).

Dear Parents / Guardians:

The SEEM Prep Program maintains in its school office copies of the following program documents:

You are welcome to visit the program and review these documents. With regards to the coronavirus (COVID-19) pandemic, scheduling a time to review the documents with the secretary is required. Photocopies of policies and procedures are available upon request.
