To review:

Each student needs to have on file, update and signed by the child's physician:

1) A signed record of immunizations

2) A second record of a physical completed in the past year (12 months)

3) A signed medical care plan, if you have received one from the nurse

All medication(s) should be delivered to the school in a pharmacy- or manufacturer-labeled container(s) by you or a responsible adult who you designate. Please ask your pharmacy to provide separate bottles for school and home. No more than a thirty (30) day supply of the medication should be delivered to the school.
