Before you can take part in Century's Dual Credit Courses, please fill out each form and deliver or email the forms to the counseling center.

Fast Forward Participation Form
Students & parents must sign the Advanced Opportunities Participation Form before being eligible for state funding. This form should be turned into the College & Career Advisors, located in the Counseling Center at CHS. There are risks to taking advanced courses. The form outlines some of the potential risks so that the students & parents are aware before they commit to the course(s).

ISU Early College Consent Form
Students & parents must sign the Early College Program Dual Credit Student-Parent/Guardian Consent Form. This form goes over the expectations & eligibility requirements for a student taking ISU Dual Credit courses. This form can either be printed & submitted to the College & Career Advisors (located in the Counseling Center at CHS) or can be completed online.
