The Central AT team can assist through AT Consultation (HIAT) or AT evaluation by submitting an AT Evaluation Referral
AT Consultation
After the consideration process and the team warrants a consultation from the Center for Assistive Technology , make an appointment with an evaluator.

Procurement Order (PO) Assistance
HIAT Follow-Up Consultation
HIAT Initial Consult with SETT Framework

AT Evaluation Referral
After the consideration process and the team warrants an AT evaluation by the Central Assistive Technology team, complete an AT Evaluation Referral Form.
