Nurse's Note

Hand washing continues to be the single most important thing that you can do to stay healthy & well. Please review with your child, proper hand washing as we begin a new school year.

Physical Examinations

It is important for students to receive medical examinations during the developmental stages of early childhood, pre-adolescence, and adolescence; therefore it is recommended that students be examined by their physicians in Grades 1, 3, 5, and 7.

Private Physical Examination Forms may be found on the school website.

Sports Physicals may be used as the private examination; however the Private Examination may not be used as the Sports Physical

School Sports

Any student wishing to tryout for a school sport must have a sports physical on file with the school nurse prior to the tryout . This includes Athletic Physical Examination Form, Clearance Form, Athletic History Form as well as the SBS Sports Related Concussion Acknowledement Form, and the Cardiac Sign Off Sheeet . The sports physical may be used for all sports that your student may tryout for during the school year, within 365 days from the date of the actual examination. There are no exceptions, as this is a state mandate .

Please check with the team coach or the school nurse for your child's eligibility status.

***********************Please Note: Eye injuries are the leading cause of preventable blindness and visual impairment in children, and most injuries occurring in school-aged children are sports-related. The fact sheet, developed by the New Jersey Department of Education, includes critical information to promote the prevention of eye injuries as well as recommendations for the appropriate management of eye injuries that may occur. Each school district and nonpublic school is required to distribute the educational fact sheet annually to parents or guardians of the students, pursuant to N.J.S.A. 18A:40-41.9(b), in order to reduce the needless loss of sight that can occur during sports activities.
