Fever over 100.4 degrees
Diarrhea and/or vomiting WITHIN THE LAST 24 HOURS
Persistent upset stomach or headaches
Persistent sore throat with fever or swollen glands
Rash with fever
Any student who sustains an injury or becomes ill during school hours should go to the Health Office for evaluation. Often what appears to be a minor injury or illness is more serious; therefore, please permit the nurse the opportunity to evaluate the situation and make the appropriate recommendation for care.
Students needing to be seen by the nurse should first obtain a pass from their teacher unless it is an emergent situation. Students needing to be dismissed from school due to illness or injury should be dismissed through the Health Office.
We ask that students do not call their parents before coming to the Health Office if a student requests dismissal. This allows the nurse to assess students and make appropriate recommendations.
The Health Office will contact the parent/guardian designated on the emergency form to obtain permission for dismissal.
OVER-THE-COUNTER MEDICATIONS
For safety reasons, all medication must be brought to the Health Office and administered under the supervision of the nurse. The student should not possess or ingest prescription (other than Inhalers or EpiPens) or over-the-counter medication in a location other than the Health Office. If students require an over-the-counter medication that is not currently stocked in the Health Office, they will need to bring in their own supply of the medication in an originally labeled bottle along with a note from their parent/guardian giving permission to administer the medication as needed.
PRESCRIPTION MEDICATIONS
If it is necessary for a student to take prescription medications during the school day, a physicians order from the prescribing physician must be brought to the Health Office. Medication must be received in the container labeled by the pharmacist. It will be kept in a locked cabinet in the Health Office. Students with asthma should carry their prescription inhalers with them. Students with severe allergies who have EpiPens should keep their EpiPens on them at all times. All controlled substances must be brought to the Health Office by a parent/guardian.

Prior to entry, all new students must present an immunization record or religious or medical exemption to the Nurse. Information on medical exemptions, and the form to be notarized, can be found on the NH DOE website: https://www.dhhs.nh.gov/dphs/immunization/exemptions.htm
Documentation of all student immunizations, as well as religious or medical exemptions, will be kept on file in the Health Office. Failure to comply with immunization requirements may result in exclusion from school.
When your child receives immunizations, please provide an updated copy to the Health Office. The Health Office will contact you if any additional immunizations are required.
