At the start of each school year the health office requires completion of the annual Emergency Health Form for every child. If the form is not completed and/or on file we will need to contact you each time your child needs OTC items (acetaminophen, ibuprofen, antacids, lip ointment, etc.)

We strive to provide the best possible care to your child. We request you inform the nurse(s) of any health problems that should be a matter of record or when your child has an illness or injury that may require assistance while at school.
