Welcome to the Chavez Check In family survey! Chavez is now a Community School! This means we want to hear from you to create change, your thoughts and experience at Chavez will help us make our school better for everyone. Your honest feedback will make change happen!

Survey Results will be Shared Out to All! The information gathered from family, student, staff and community surveys will serve as a roadmap to programs, systems and programs that will support the entire Chavez community.

Chavez Check In Update: We've gotten 91% of our family surveys turned in and 40% of student surveys completed! Thank you to all who've supported this milestone. It's not too late to submit your family survey! Your honest and intentional answers are highly valued and will be very important in creating the positive change and support for the Chavez community. Once all has been gathered and reviewed, a graph and flier with the results of all surveys will be shared out to the community!

Every student who's family has turned it in will receive one free Kona Ice snow cone on the 14th!

Family & Student Survey Data Update as of January

As of January, we have 91% of family surveys sent in and have created graphs and posters to view below with the information! We also have created graphs of student answers and as of December have 40% of student information. We are still working on interviewing every student at Chavez and getting their opinion about what they would like to see here too. We are still working on gathering student and family surveys and the information below that reflects their responses will be updated in the coming weeks. Be sure to check back for the final results of all surveys collected as well as staff and community partner survey data too. Equal voice and collaborative leadership is so important in this process, we highly value family, staff and student input!

Implementation Plan

What is the goal of Community Schools and how is it reached?

Goal: Hear from all community members (educational partners) to better understand the needs, wants, and assets of the community as a whole.
Purpose: Identify the most important areas of growth/need in the community in order to form working groups of interested educational partners who can engage in direct action as well as the allocation of resources to address the prioritized need.
Process: Engage in a series of focus groups as well as surveys and one-on-one conversations to ensure multiple data-points are garnered from all educational partners (students, parents, staff/teachers, and community partners) regarding their chief concerns for the community as well as highlight actions that they may be willing to take within the community schools work.
