1. Provide a COPY of your Child's Baptism Certificate to the Parish Office, attn: PSR Registration by mail or drop- off to 525 Dover Center Rd. Bay Village, OH 44140

3. Complete Health/Medical and Media/Virtual Releases by completing this online form
