Dear Parents/Guardians, Summer 2025
Greetings from the Elementary Health Office! We hope this summer has been restful and productive for you and your families. This letter outlines our documentation requirements and infection prevention procedures for the coming school year.
PHYSICALS AND IMMUNIZATIONS:
As of July 1, 2018, New York State law requires a health examination for all students entering the school district for the first time and when entering Pre-K or K, 1st, 3rd, 5th, 7th, 9th, and 11th grade. Those students entering Pre-K, kindergarten, sixth, seventh and twelfth grade will need new immunizations which will be required by the 14th day of school. Please call and schedule your appointment today.
The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner within the past 12 months, so we can accept any physical done after 9/03/24.
A copy of the health examination must be provided to the school within 30 days from when your child first starts at the school, and when your child starts K, 1st, 3rd, 5th, 7th, 9th, & 11th grades. If a copy is not given to the school within 30 days, the school will contact you.
Pediatricians can print the necessary paperwork for you. Please be sure to include an updated immunization record.
If your child has an appointment for an exam during this school year that is after the first 30 days of school, please notify the Health Office with the date.
Communication between private and school health staff is important for safe and effective care at school. Your healthcare provider may not share health information with school health staff without your signed permission. Please talk to your provider about signing their consent form for the school at the time of your child’s appointment for the examination.

OVER THE COUNTER MEDICATIONS:
Occasionally, your child may benefit from medication at school. For example, ibuprofen can help with swelling or pain after an injury. The school nurses CANNOT administer medication without a written signature from your healthcare provider. We will always contact you for your verbal permission prior to administration of any medication, but we can not give it unless we have the permission form on file.
ALLERGY AND ASTHMA FORMS:
If your child has an allergy that may require epinephrine or antihistamines, please send an allergy action plan and signed permissions to the health office.
If your child has asthma, please fill out the asthma forms, and send in any written permission for administration of inhalers at school.
WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL:
Please do not send your student to school with any of the following:
● Fever over 100°F - Must be fever free for 24 hours before returning to school, without the use of fever reducing medications like Tylenol or Ibuprofen
● Vomiting or diarrhea - Must be 24 hours without vomiting or diarrhea to return to school
● Conjunctivitis (“Pink Eye”)
● Sore throat
● Strep throat - May return to school after 24 hours of antibiotic treatment
● Bad cold, with a very runny nose or bad cough, especially if your child was awake at night
If a student becomes ill at school or is contagious to other children, parents/guardians will be called to take them home. Please be sure that you have arrangements to transport your child home should this occur. If your daytime or emergency contact numbers change during the year, please update them on the Parent Portal on eSchool or call the main office.
TICK REMOVAL, SUNSCREEN AND INSECT REPELLANT:
School nurses need permission from parents and guardians to remove embedded ticks, apply sunscreen and apply family provided insect repellant. Please help us by filling out the following google form:

HEARING, VISION AND SCOLIOSIS SCREENINGS:
The Sag Harbor Union Free School District’s Health Services program helps support your child’s academic success by promoting health in the school setting. One way that we provide care for your student is by performing the health screenings as mandated by the State of New York. During the school year, vision and hearing screenings will be completed at school for those students who are new to the Sag Harbor Union Free School District, as well as students in PreK, Kindergarten (those not screened in preK), Grades 1, 3, and 5. Screenings will begin in October. Scoliosis screening (spinal curvature) will be performed on 5th grade girls as well. Students with documented complete hearing, vision or scoliosis results from their pediatrician may not be screened. A letter will be sent home with the screening results and will note if your student needs to follow up with their healthcare provider.
If you have any questions or you would prefer for your child to be screened with your private pediatrician and not at school, please email your school nurse. We will send you the screening form for your child’s physician to complete.
HEALTH CONDITIONS AND WEIGHT STATUS REPORTING
As part of a required school health examination performed by your healthcare provider, a student is weighed, and his/her height is measured. These numbers are used to figure out the student’s body mass index or ‘BMI’. The BMI helps the doctor or nurse know if the student’s weight is in a healthy range or is too high or too low. Recent changes to the New York State Education Law require that BMI and weight status group be included as part of the student’s school health examination. A sample of school districts will be selected to take part in a survey by the New York State Department of Health. If our school is selected to be part of the survey, we will be reporting to New York State Department of Health information about our students’ weight status groups. Only summary information is sent. No names and no information about individual students are sent. However, you may choose to have your child’s information excluded from this survey report.
The second part of this survey includes a tally of how many students in the selected grades have chronic health conditions like asthma or diabetes. Again, no names or demographic information are sent, just a total number of students with each condition are noted.
The information sent to the New York State Department of Health will help health officials develop programs that make it easier for children to be healthier.
If you do not wish to have your child’s health condition or weight status group information included as part of the Health Department’s survey this year, please contact your school nurse.
