The Transition Improvement Grant (TIG) is designed to strengthen and accelerate the transition process in our schools to increase high school graduation rates, decrease dropout rates, improve the quality of postsecondary transition plans, and develop strong career, college, and community readiness among our students with disabilities through positive post school outcomes.
The Transition Improvement Grant has designed a transition app to increase self-determination and self-advocacy for all Wisconsin youth in the transition planning process. This app directly aligns with the Postsecondary Transition Plan (PTP) within the students Individualized Transition Program (IEP) TIG is excited for youth, educators, families, and agencies to begin using this application!
This app is designed to help Wisconsin students and family members participate in the transition planning process and prioritizes the student’s preferences, interests, strengths, and needs. It is a powerful tool that can increase engagement of the student and their families in transition planning discussions. The questions in the app help students provide input into their postsecondary transition plan requirements of the state and the Individuals with Disabilities Education Act (IDEA).

Each summer Wisconsin school districts conduct a telephone interview with families of students who received special education services while they were in high school and graduated or left school during the previous school year. Answers will provide valuable information that helps districts make improvements and plan classes and activities for future students, and it will help adult service agencies provide better services to young adults. The questions only take about 5 minutes to answer and participation is voluntary. Individual answers will not be shared with anyone.
Sometime between June 1 and September 30, 2021 a representative from your school district will call you to ask you some questions about the types of employment and continuing education you have participated in since leaving high school. Comments about work, school or living experiences can also be shared.
