Aging & Disability Resource Center (ADRC)
The ADRC helps individuals plan and make informed decisions through an interactive decision support process. This includes a discussion of the individual’s wishes, needs and goals in order to help the individual identify and evaluate options available.
Options counseling includes the following:
A range of community resources and service options available to the individual, including but not limited to: home care, housing, nutritional resources, community services, residential care, nursing home care, and case management services. Factors to consider: services, cost, quality, compatibility with individual preference, goals and outcomes.
Opportunities and methods for maximizing independence and self-reliance, including the utilization of supports from family, friends and community and the self-determination approach.
The sources and methods of both public and private payment for long-term care services, including: information about the long-term care programs available in the area, such as Family Care, IRIS, Partnership, and other programs for which the person may be eligible.

Long-Term Care & Support
Long-term care is any service or support that you may need because of a disability, getting older, or a chronic illness that limits your ability to do the things that are part of your daily routine. This includes things such as bathing, getting dressed, making meals, going to work, and paying bills.
Long-Term Support: Family Care
Family Care is a program that provides a full range of long-term care services, all through one flexible benefit program. Members of Family Care enroll in a managed care organization to receive their services. The organization provides and coordinates services for its members.
Long-Term Support: IRIS
IRIS (Include, Respect, I Self-Direct) is built on the principles of self-determination and self-direction. That means that you will have the freedom to decide how you want to live your life.
When you join IRIS, a budget is established for you based on your long-term care needs and goals. You get to decide what goods, support, and services will help you achieve your goals and, using your budget, you will help create a support and service plan to meet your needs.
