Immunization Records & Physicals
ALERT!! GRADE 5 STUDENTS!!
A physical is required for entry to Rockwood Grade 6. Please consider using the holiday breaks to secure your appointment for a physical & vaccinations!!
You may turn this information in to Nurse Eli.
If you have a student entering Kindergarten, Grade 6, or a student new to Rockwood, REGARDLESS of whether that student is attending in-person or online, that student must have a physical and updated immunizations turned in BEFORE the first day of school, August 23, 2022. Please have your physician fax these records to Nurse Eli fax: 636-891-8862, or email a copy to Nurse Eli: brueggemannelissa@rsdmo.org. Update your child's health & medical information:
Health & Medical information must be updated online. Log onto your student's Infinite Campus account to update. It is very important to update this information so your student may be properly cared for in the event you are unavailable. If you do not have access to your student's Infinite Campus account, a form may be requested from the main office. Thank you for updating your student's information.
