Prescription Medications
These are medications that are prescribed by your child's doctor, for example: asthma, epi pen, diastat, ADHD medications.
Parents print and fill out the prescription medication forms needed for your child (see "forms needed" above).
Take this form to your child's doctor to be completed.
Parent signature is also needed on the form.
Medication forms need to be completed every school year.
Prescription medication needs to be given to the School Nurse in its original package from the pharmacy, and it must have your child's name and pharmacy label on it .
Over the Counter (OTC) Medications
Medications you can buy at the store without a prescription, for example: ibuprofen, cortisone cream, allergy medication.
Parents print and fill out the over the counter medication form (see link below) and return it to the school nurse with your brand new, unopened medication.
If over the counter medication(s) need to be given more than 3 days in a row, a doctor's order will be needed.
