4. District Transfer Application 2024-2025
This link is for students that live outside of the CHS boundaries, but would still like to attend our school. This form needs to be submitted every year for approval by District Office. Please click on link and follow the directions. **Make sure to apply for the correct school year when applying*
If you AND your student live in a residence with a friend or family member, you and the property owner will need to fill out an Address Verification Form and get it notarized. You can then turn both that and the owner's proof of residency to us at the front office or through the above Document Upload Link. This is something that has to be renewed each year.

If you are in need of an official transcript please click on the link above to complete and order. In order for the transcript to be OFFICIAL, please send it directly to the college/university or employer. If it is sent to the student first, it will not be accepted as official.
