Step 1: Notice of Intent to Participate Form

By law, a resident district must be informed if a student intends to participate in the choice program.  Therefore parents/guardians of students who intend to participate in the choice program as of the 2024/2025 school year must print and complete the Notice of Intent to Participate Form shown on this page;  submit it to your resident district by Friday, November 17, 2023; and obtain a signed receipt acknowledging that you submitted the form.  Keep the receipt for your records.
