Counseling Support Request Form
Do you need support from your counselor? You have come to right place. The school counselor provides academic, college/career, and social/emotional support to students at the request of students, parents, and teachers/staff. The following is a non-comprehensive list of supports that the school counselor offers:
If you would like to request an appointment with the counselor, you may:
1. Fill out a counseling slip and drop it in the locked box on the counselor's office door.
2. Fill out the following form.
This form can be filled out by a student, teacher/ staff or parent. Once the form is filled out, we will reach out to you to set up an appointment.
