Incident Report Procedure
Initiator
Student Incident Occurs
Provide care for the injured student/visitor
Notify the parent of the Incident, noting the time the parent was notified
If the student is referred to the nurse or athletic trainer, you may defer parent notification to the individual providing treatment
Notify the Athletic Director or Administrator
Complete the Incident Report in Informed K12 with as much information as possible by the end of school day/game/practice.
The employee/volunteer that witnessed the incident MUST complete the report.
If an employee did not witness the incident, the employee/volunteer responsible for the student MUST complete the incident report.
Send the incident Report to the school nurse where the student attends school.
If the injured person is a visitor, send the form to the school nurse where the incident occurred.
When is an Incident Report needed:
Fill out an Incident report if a kid is injured on campus or during a campus activity.
Examples of when to complete a report:
Called 9-1-1
Occupational hazard (ex: student injured by object at school/during practice)
Broken bone
New seizure (no history of seizures); which would require 911
