Athletic Clearance

A new physical must be completed each school year, prior to participating in any conditioning, tryouts, practices, games, or summer conditioning.

 Physicals for the 2023-24 school year MUST be dated ON or AFTER May 15, 2023. 

Physicals may be completed by your Primary Care Physician, a Drop-In Clinic, Physician Assistant, Nurse Practitioner, or Doctor of Osteopathic Medicine only.  RJUHSD does not accept athletic physicals completed by Chiropractors. 

Please allow at least 72 hours for processing. 

BEFORE YOU START the online Athletic Clearance 

1. Please clear all fees/fines/book returns. 

2. Physical Forms must be updated each school year for your physical appointment. All four forms must be uploaded and are easiest to upload two at a time. Physicals for the 2023-24school year MUST be dated ON or AFTER May 15, 2023. See below to print forms.

3. Have your student athlete's insurance card ready to upload. If you do not have medical insurance, you may purchase it by following this link. You must have insurance to participate. If you do not have an insurance card with your child's name on it, you will need to get a letter/email from your insurance company that lists your covered dependents.
