Physical Forms must be updated each school year for your physical appointment. All four forms must be uploaded and are easiest to upload two at a time. Physicals for the 2024-25 school year MUST be dated ON or AFTER May 15, 2024. See below to print forms.
Have your student athlete's insurance card ready to upload. If you do not have medical insurance, you may purchase it by following this link. You must have insurance to participate. If you do not have an insurance card with your child's name on it, you will need to get a letter/email from your insurance company that lists your covered dependents.
