Directions

1. Complete instruction on lessons # 1-8.

2. Students finish their Transition Guide.

   • • 9th-11th Grades: Part 1 Only

     • 12th Grade: Parts 1 & 2

3. Teachers will calculate totals from student Transition Guide.

   • • Alternately, teachers can create a Google Forms Survey and have students migrate the information from the Transition Guide into the Google Forms Survey.

4. Teachers will submit the results of the data collection to the Coordinator Principals or Administrative team.
