This form is for low and medium risk activities.
Make a copy of this plan. Name it with the event and date. Share with the DP's or EOTC coordinator. Once approved please print two copies, one to take with you to the event and one to sign and leave at the office with a list of all children participating in the event.
This link takes you to the page to access all 18 of the EOTC forms. High risk (e.g. surfing, abseiling) and over night events must use the #2 EOTC Risk Assessment form. High risk and overnight events also require completion of an #1 Event Proposal form and also #17 Review form to be completed after the event. Many of the other 18 forms are also required as appropriate for the event (e.g. Health Profile, Parent Consent etc.)
