1. How should we scan/submit the NHS application when it is completed?
You will complete the application using Google Docs. After you have entered all the information about your grades and clubs, etc. you will print it out and get physical signatures from the club advisors. After you have all the signatures you can go to the library and scan the printed out application to a PDF. You will then use the Google Form on the NHS website to submit the scanned PDF.
2. Should a NHS applicant physically be giving Ms. Penta their two recommendation letters?
All NHS letters of recommendation need to be submitted electronically. An email was sent to the high school staff with instructions on how to complete a recommendation letter for prospective members.
If you would like to have someone from outside the high school staff write a letter of recommendation for you, please ask them to email Ms. Penta at lpenta@pvcsd.org and she will email them the electronic form they need to complete.

5. Why can't I access the NHS application?
Please use your school Google account because all students have access to the NHS application form. If you can't access it, then please make sure you are not using your personal Google account.
6. What should I do if it is not possible to obtain the signature of a former coach/teacher (not at PVHS)?
You can do ONE of the following:
1. Scan the application that requires the signature and email it to that person.
OR
2. Take a picture of the NHS application and send the application by way of text or email.
This person will be responsible for printing, signing and sending the application back to you in the same format.
7. What if I can't get a signature because the person is unresponsive or no longer available?
First, speak to or email Ms. Penta to let her know about the situation. Next, on the application please note in the signature box the reason why you can't get the signature.
8. Can Ms. Penta, the NHS advisor, write a recommendation letter for me?
No, Ms. Penta cannot ethically write a letter of recommendation because she is the NHS advisor and that would be a conflict of interest.
