Diversity, Equity, Inclusion and Belonging Recommendation and Request Form 

The Purdue Global Office of Organizational Culture and Inclusive Excellence (OCIE) utilizes this form to bolster our diversity, equity, inclusion, and belonging (DEIB) initiatives for PG employees. We welcome all DEIB-related requests or referrals, such as requesting a Pronoun Change in Brightspace, modifications to EEOC job applications, or Chosen name changes. These examples illustrate the types of submissions we've addressed previously, but we're open to all DEIB-related inquiries.
