Homicidal / Suicidal Protocol for School Staff
Whenever a student makes any kind of statement that involves danger to self, we must always assess lethality. We must determine how serious the thoughts and behaviors of the student are. To assess if a student is serious, we must ask questions. Questions are focused on determining time, plan and means: a time frame to hurt themselves, a specific plan on how they would hurt themselves and do they have the means in which to carry out the plan.
Sample questions for Suicidal/Homicidal Ideation:
- Do you have a specific plan to harm yourself or others?
- How long have you been thinking about this?
- How soon will you hurt yourself or someone else?
- Do you think that you will be safe from harming yourself or others within the next 24 hours? 48 hours?
- Do you have access to make your plan a reality? (Does the client have access to a gun, pills, etc.)
- How often do you think about harming yourself or others?
- On a scale from 1 to 10, with 10 indicating that you absolutely want to harm yourself right at this minute, and 0 indicating that you have no intention at all in harming yourself, where would you place yourself?
- Have you ever tried to harm yourself in the past? How? How often?
- Have you ever talked to anyone about these feelings? What happened?
If a student is not serious threat to themselves but are depressed, please refer to the counseling department.
If a student has expressed a specific time, plan, and means for homicide or suicide you must CONTACT YOUR SUPERVISOR IMMEDIATELY. Seek assistance from the Counseling Department - either a counseling trainee, Clinical Supervisor or Director of Clinical Services.
Do not leave the student alone - approach a team member, counselor or school administrator to assist you in supporting/supervising your student.
If a Hospitalization Assessment is needed CALL PMRT: (800) 854-7771
Call the PMRT (Psychiatric Mobile Response Team) so that your student be assessed for hospitalization.
Before you call the PMRT, have the student file in your possession.
After you call the PMRT, call the parent/guardian, explain the situation, and ask them to come to school to be part of the assessment.
If the parent and PMRT assessor decide that hospitalization is in order, once the 5150 hold has been written, the student is now under the jurisdiction of the LA County PMRT.
If the team decides that the client doesn’t meet criteria for a 5150 hold, the client is released into the care of the parent. The Counseling Department will follow up with the student.
CALL 911:
If at any time during this process, your student escalates and becomes overtly dangerous to self or others, call 911 immediately!
If your student discloses that he/she has ingested a poison or attempted to overdose on medication, DO NOT CALL THE PMRT- CALL 911 IMMEDIATELY (even if your client is lucid and appears normal).
Call 911 from a landline vs. a mobile phone- the response time will be much faster.
