Please be informed that this website and its related forms and programs, will be collecting personal information and other related data which will be used for enrollment and other academic purposes.

Who are authorized to handle your personal information?

Current Faculty Members and Staff of the College of Sciences are authorized to collect your personal information. Selected Organization Officers, with strict supervision from their Faculty Advisers, are also authorized to collect basic data, just for the purpose relevant for their respective organizations.

Basic personal and contact information.

What would be the purposes of data processing activities?

Education and other university and college-related purposes.

With whom will we share your personal data?

Your personal data will be shared to other University Officials upon request and only upon approved request.

Where will we store your personal data? How will we protect your data?

Currently, your data is stored in two means: through the Cloud Storage of the University through Faculty Officials, and in hard-copy versions, stored securely at the Dean and Chairperson's Office.

How can you access and correct your personal data?

You can access your data anytime by contacting the appropriate college/university official at the appropriate offices. Any updates or corrections to your personal data are welcome.

This Notice is effective on July 16, 2020, and may be amended from time to time, with such amendments to be reflected here.
